  • Advanced Health Care Directives

    • This document allows you to give instructions about your own health care. It allows you to name another individual as agent to make health care decisions for you if you become incapable of making those decisions or if you want someone else to make those decisions for you now even though you are still capable. This document also allows you to give specific instructions for your own health care wishes and lets you express an intention to donate your bodily organs and tissues following your death.


  • Grant Deeds

    • A Grant Deed transfers the title of real property from an individual person(s) to his/her/their trust.This ensures that the assets are titled correctly to fund the trust.A Preliminary Change of Ownership Report (PCOR) is filed with the Grant Deed to prevent an increase in property tax.


  • HIPAA Authorizations

    • This document allows the identified persons to obtain protected health information on your behalf in order to make informed decisions about your care and the payment of medical bills.


  • Powers of Attorney

    • In your “Power of Attorney,” you appoint agents to do anything you want your agents to do for you should you become unable to effectively manage your property or financial affairs.


  • Pour-Over Wills

    • A Pour-Over Will is a document that acts as a safety net for a Revocable Living Trust (RLT) and catches any property or assets not transferred directly to the trust prior to the trustmaker’s death.
